\ndose.
\n\u2022\tIf this is your child’s first dose, please register for one slot and
\nyou will automatically be enrolled for the second dose on February 2nd.
\n\u2022\tIf this is your child’s second dose, please register for a slot.
\n\u2022\tPlease bring both the registration form and the consent form to both
